Statewide and Local Forms

There are two different types of forms that you may need for your case. One type of form is a Statewide form and the other is a Local form that is only used in your county.

Statewide Forms

Statewide forms are usually available for most cases and can be filled out to decribe what you are requesting from the court. You can sort by the type of forms you desire, and the category.

Local Forms

Local forms are usually filled out in addition to statewide forms. They may be required along with your forms when you file them at the court, or they may be able to be filed afterwards. it is a good idea to ask the clerk at your local court, since requirements can vary.
